Report of Changes in Alien Registration Matters
Under Article 35 of the Immigration Act, registered aliens [except for those subject to Report of Domestic Place of Residence (F-4)] are required to report changes in Alien Registration Matters set forth below to the Chief of the Immigration Office having jurisdiction over the place of sojourn within 14 days and administrative fines are imposed for violations of Article 35 of the Immigration Act.
Matters to be Reported : Name, Gender, Date of Birth, Nationality, Passport Number · Date of Issue · Expiry Date
