Application for Change of Department for Enrolled students, Spring 2021

 

This is to notify you of information about application for change of department for Enrolled students for Spring semester, 2021. Those who want to change the department are required to submit the application form within the designated period stated below.

 

1. Qualification for application

 a. Enrolled students who completed one or two semesters at the said graduate school

 b. For any of the degree courses (Master's, Ph.D and Integrated Courses)

 

2. Required documents

 a. Application form for change of department (Prescribed form) : Attachment #1

 b. Application form for Credit Recognition from the change of department : Attachment #2

 

3. Place for submission : Department office for moving in (Department office that you want to join)

 

4. Application Period for change of department : Until Jan 27th(Wed), 2020

 

5. Notes

 a. The number of application for change of department : Only once while in school.

 b. Confirmation of the change of department and school register will be reflected as of March 1st, 2021.
