REQUIRED DOCUMENTS SUBMISSION FOR SUMMER SEMESTER 2022 (GOOGLE FORM)

All residents of the Ajou dormitory for the summer semester 2022, please submit the relevant documents via the link below.

1. TUBERCULOSIS TEST RESULT

   1) Subject: Prospective residents who have not submitted TB test result to the dormitory officer OR who had submitted the TB test result more than 1 year ago from July 3rd 2022 (Move in date).

   2) TB is valid only for 1 year.

   3) The result sheet must show either normal or abnormal of TB in English or Korean.

   4) File name should include studentIDnumber_name. e.g. 2022XXXXX_REBECCA

2. RAPID ANTIGEN TEST OR SELF TEST KIT

   1)If you are a student who stayed for spring semester 2022, you need to do a test from 30 June to 3 July.

   2) If you are a new resident for summer semester, you need to do the test within 2 days from your move in day. (e.g. move in day – 3 July / test conducted – 1~3 July)

   3) File name should include studentIDnumber_name. e.g. 2022XXXXX_REBECCA

   4) If you have difficulties in conducting a rapid antigen test, you can test with the self test kit (Your name and test date must be written on the kit). Update the picture of test kit.

   5) Do not cheat the test date for the safety of other residents. All prospective residents have responsibilities not to spread the COVID 19.

3. STATEMENTS OF SUPERVISOR, COMPLETION CERTIFICATE

   1) If you are a graduate student who completed the course works, you need to submit above documents.

   2) You can ONLY apply to the dormitory when you submit these documents.

   3) You can only stay in the dormitory up to 6 semesters from the completion date of coursework.

   4) If there are no enough capacity in the dormitory, your application will not be approved even though you submit all required documents.

   5) You need to combine two copies to One PDF file.
